In-Home and Community Support

In-Home and Community Support helps individuals build the skills they need to live more independently and take part in everyday community life.

Services are provided at home and in inclusive community settings. Support may include hands-on assistance, teaching, guidance, and encouragement with:

  • Personal care and health

  • Decision making

  • Home and money management

  • Communication and social skills

  • Transportation and mobility

  • Community activities and resources

Each person’s support plan is based on their unique needs, preferences, and goals. Our focus is on promoting independence, confidence, and meaningful community involvement.

Community Participation Support

Community Participation Support helps individuals get involved in their community while building skills and confidence for future employment.

This service offers opportunities to explore interests, discover strengths, and develop abilities in real-world settings. The goal is meaningful participation in a wide range of inclusive community activities—alongside others—not in separate settings.

Support is based on each person’s unique interests, preferences, and goals. Individuals are encouraged to:

  • Explore hobbies and interests

  • Build social and communication skills

  • Gain confidence in community settings.

  • Develop strengths that can lead to competitive employment

  • Become active, valued members of their community
